Since 1985

Our Adult Day Health Center offers an array of supportive services designed to prevent or postpone the inappropriate institutionalization of frail, homebound elderly or functionally limited adults conveniently located in Webster, Massachusetts.

The adult day industry is comprised of two models of care, the social model and the medical model. The social model of care, are community-based group programs designed to meet the needs of functionally and/or cognitively impaired adults through an individual.

Accord Adult Day Health Center is certified to provide Adult Day Health (ADH) Services by the Commonwealth of Massachusetts, Department of Health and Human Services, Executive Office Of Elder Affairs and the State of Connecticut, Department of Social Services.alized plan of care. These structured, comprehensive programs provide a variety of social and other related support services in a protective setting during any part of a day, usually between four and nine hours per day. The medical model incorporates all the features of the social model but adds a wide range of health related services on site.

 

Model of Care

Accord Adult Day Center is designed to meet the needs of functionally; cognitively impaired and/or isolated adults and seniors.  Our structured programs provide a variety of social, physical and medically related services and interventions, in a protective and stimulating setting. We coordinate the participants’ individualized plan of care with families, caregivers, their primary care physician and other involved support agencies.

Our dedicated professional staff provides the participants with individualized assistance of daily living and medical interventions that they require; under the direction of their own primary care physician.
